Job summary

Post-Acute Rehab Solutions Therapy is seeking a committed Home Health Physical Therapy Assistant in the West San Antonio, TX area. The PTA will collaborate with a licensed PT to implement patient-centered plans and promote mobility, function, and quality of life in the home setting.

Responsibilities include delivering evidence-based interventions, documenting progress, and participating in multidisciplinary care. CPR and TX PTA certification are required, with home health experience preferred.

Qualifications

• PTA certification in TX is required.
• CPR certification required.
• Experience in home health settings preferred.
• Ability to collaborate with a multidisciplinary team.
• Strong communication and patient education skills.

Responsibilities

• Provide PT assistant services under licensed PT supervision in home health.
• Identify and meet patient goals and treatment plans.
• Develop care plans and tailor programs for mobility and function.
• Conduct progress evaluations and document care provided.
• Promote safe, cost-effective treatments and patient education.
