Job summary

MorningStar Senior Management LLC is seeking a compassionate Wellness Nurse (LPN or RN) to support residents in our Old Town senior living community. This on-site role provides high-quality clinical care, administers medications, monitors health, and supports individualized care plans.

You will collaborate with caregivers, families, and providers in a faith-inspired, servant-leadership environment that honors seniors, with ongoing training and growth opportunities to make a meaningful daily

Qualifications

• Active LPN or RN license.
• BLS certification and First Aid/CPR.
• Experience in assisted living or senior living.
• Knowledge of medication administration regulations.

Responsibilities

• Provide direct nursing care and wellness support to residents in assisted living and memory care.
• Administer medications safely and accurately per physician orders and community policies.
• Monitor, document, and report changes in resident condition using EHR systems.
• Implement and support individualized care plans in collaboration with interdisciplinary team.
• Communicate effectively with residents, families, and healthcare providers.
• Promote infection control, fall prevention, and safety protocols throughout the community.
• Support, mentor, and collaborate with caregiving staff to ensure quality care and service.
• Participate in training, audits, and quality improvement initiatives to enhance resident outcomes.
• Vital signs monitoring
