Job summary

ProMedica in Ohio is seeking a part-time Registered Nurse to support the Annual Wellness program. The schedule is Monday and Tuesday full days, plus a half day on Wednesday, with travel to designated practices as needed.

You will review health information, document in the EHR, educate patients on preventive services, coordinate referrals, and work with physicians and staff to improve outcomes. Requires Bachelor's in Nursing, RN licensure, CPR, and at least three years of clinical nursing

Qualifications

• Bachelor's degree in Nursing from an accredited school.
• 3 years of clinical nursing experience.
• Active RN licensure in state of assigned practices.
• CPR certification.
• Proficient in Microsoft Office and EHR systems.
• Ability to work flexible hours and multitask.

Responsibilities

• Gather and review eligible beneficiary's health information.
• Update medical, social and family history in records.
• Review Health Risk Assessment to identify barriers and risks.
• Counsel on age-appropriate preventive services.
• Provide personalized health advice and referrals as appropriate.
• Update the EHR with current information.
• Maintain positive relationships with physicians, staff and patients.
• Engage patient in self-management and education.
• Document encounters timely in the EHR.
• Collect data, track outcomes, and support planning.
• Attend relevant conferences and trainings.
• Travel to designated practices as needed.
• Participate in quality improvement initiatives.
• Complete special projects as assigned.
