Cogir Senior Living in the United States seeks a Wellness Nurse (LPN) to collaboratively provide health assessments, medication management, and coordination with residents, families, and the care team. A valid LPN/LVN license and at least four years in geriatric nursing are required, with on-call rotation expected.

Qualifications

  • Current First Aid and CPR license or ability to obtain.

Responsibilities

  • Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
  • Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
  • When necessary, participate in hands-on care, medication administration, and specific nursing tasks.
  • Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines.
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
  • Assist in the transcription of physicians' orders and the review of new physicians' orders.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with home health agencies on-site, working within community and company policies.
  • Address residents’ special needs, requests, and complaints.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Understand the community's care regulations along with state-specific regulations.
  • Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.

Position Summary

Our Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our residents. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Requirements
CANDIDATE QUALIFICATIONS
Education And Certifications
  • A valid, incumbent nursing license (LPN/LVN) is required.
  • High School Diploma or equivalent.
  • An associate degree or higher in nursing is preferred, or a combination of education and experience. Additional coursework in management is an asset.
  • Current First Aid and CPR license or ability to obtain.
Experience, Competencies, And Skills
  • At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
  • At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.
  • Experience with memory care is a plus.
  • Excellent attention to detail and high integrity.
  • Excellent communication skills and teamwork mindset.
  • Strong leadership and team management skills.
  • Outstanding organizational skills.
  • Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
  • Ability and willingness to participate in on-call rotation.
  • Must meet all mandatory health requirements according to state regulations
