Job summary

The Bungalows at Riverchase, part of Phoenix Senior Living, is seeking a Wellness Nurse to join the team. The role reports to the Wellness Director and focuses on monitoring residents' health, administering medications, and coordinating care with families.

Qualifications include LPN or RN licensure, a preference for 1 year of elder care, and the ability to work weekends/evenings. Strong leadership, documentation, and knowledge of assisted living/dementia care are required.

Qualifications

• Must be a Licensed Practical Nurse or Registered Nurse.
• Minimum of one year of experience working with elders or disabled individuals (preferred).
• Must have demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing (preferred).
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Monitor residents' health and wellbeing and provide leadership support.
• Assist with medication care administration.
• Communicate with resident support services and families; ensure quality assurance.
• Lead and support wellness components per PSP and monthly wellness visits.
• Assist Wellness Director with monthly wellness visits and PSP documentation.
• Provide direct resident care as needed.
