WellQuest Living in Pasadena, CA seeks a Wellness Nurse to deliver person-centered care, monitor resident health, and coordinate with physicians and families. The role includes documenting care plans, administering medications within scope, and participating in rounds to ensure safety and quality.

The nurse must follow regulations, maintain confidentiality, and support the dignity and respect of all residents while adhering to community policies and procedures.

• Implement and evaluate plan of care and ensure supplies and equipment are available.
• Notify supervisor when supplies are needed and respond to concerns.
• Ensure resident safety and proper shift handoffs with staff.
• Direct caregivers to ensure care aligns with standards and policies.
• Round with caregivers to ensure unit is in order and compliant.
• Adhere to Resident Rights, safety, dignity, and respect for residents.
• Maintain confidentiality and participate in meetings and trainings.

As a Wellness Nurse position is to provide Person-Centered Care consistent with accepted standards and expectations of WellQuest Living and state regulations. The Wellness Nurse responsibilities include, but are not limited to, monitoring resident health status, providing physical care, early diagnosis, and intervention of resident changes in health status. Wellness Nurses must communicate politely and effectively with family and primary physicians and perform all administrative charting or other services as required by regulations and company policy.ESSENTIAL JOB DUTIES AND RESPONSIBILITIESDedicated to delivering a high level of customer service. Provides resident care in accordance with accepted standards of practice and within the scope of the LPN or RN license.Routinely observes and reports on residents’ conditions/changes and then documents in accordance with community policies and as required by regulations.Administers medication/treatment as prescribed within the LPN or RN scope of practice.Receives, transcribes, and executes physician orders.Implements and evaluates residents plan of care and identifies and secures equipment and supplies. Notifies supervisor when supplies are needed. Ensures appropriate and timely response to resident and/or family concerns.Responsible for the safety of residents under your supervisionInforms appropriate staff about the condition of residents and expectations/needs for the shift assigned at the beginning of each shift and receives report from coworkers throughout and at the end of each shift regarding resident conditions.Directs caregivers to ensure care provided per standards of practice and per community policies and regulations.Rounds with caregiver prior to end of shift to assure unit is in proper order. Performs other duties as assigned.Adhere to the Resident Rights and promotes safety, dignity, and respect among all residents.Ensure safety of residents through appropriate behavioral interventions.Promotes employee relations, facilitates communication and problem solving.Assists with immediate incident reporting and analysis of incidents when appropriate.Maintains confidentiality of community and resident information.Attend community meetings and in-service training as required.Know the emergency plan, fire evacuation routes, extinguishers, and other facility safety features.Cart and chart audits as scheduled.
