• Perform Time and Go test and document in EMR.
• Conduct functional assessment and document findings.
• Perform mini cognitive screening as directed.
• Use EMR messaging to coordinate care with PCC/CCM programs.
• Discuss advance care planning and assist with Advance Directive.
• Obtain AWV registry patient lists and maintain EMR proficiency.
