Job summary

MorningStar Senior Management LLC seeks compassionate LPNs/RNs to provide high-quality clinical care for residents in our senior living community. The Wellness Nurse completes assessments, administers medications and treatments, monitors chronic conditions, and responds to urgent needs on NOC shift.

Qualifications

• Active Colorado LPN or RN license in good standing.
• Current BLS/CPR certification.
• Experience in senior living, long-term care, or assisted living preferred.
• Knowledge of state regulations for assisted living and memory care.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care, including assessments, treatments, and medication administration for residents on NOC shift.
• Monitor resident conditions, respond to changes, and coordinate follow-up with families and providers.
• Develop, implement, and update individualized care plans with the interdisciplinary team.
• Maintain accurate and timely documentation per regulatory standards and policies.
• Supervise, coach, and support caregivers to ensure high-quality resident care.
• Promote resident safety, dignity, and infection control practices.
• Communicate effectively with residents, families, and team members to support trust.
• Participate in rounds, care conferences, and shift handoffs to ensure continuity of care.
• Support the community’s faith-inspired, servant-leadership culture and staff education.

Partner with care teams to develop and update care plans, document accurately, and communicate with families and providers. Support a faith-inspired, servant-leadership culture by mentoring caregivers, promoting dignity and safety, and ensuring compliance with state regulations and company policies.
