Job summary

Bridge Senior Living LLC is Hiring a Wellness Nurse to implement and coordinate care for residents in collaboration with physicians and care staff. The role requires weekend availability and holiday shifts to ensure continuous care for our senior residents.

Qualifications

• LPN license in applicable state is required.
• First Aid and CPR certification is required.

Responsibilities

• Assess, plan, implement, and evaluate resident care.
• Provide high level of care by performing rounds and documenting care.
• Assist the Director of Wellness in developing and updating resident care plans.
• Maintain ongoing communication with residents, families, and physicians.

Wellness Nurse Job Summary:

Wellness Nurse will implement and coordinate the delivery of care in collaboration with physicians and care associates. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

• Assessment, planning, implementation, and evaluation of our residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents’ overall condition and behavior.
• Assist the Director of Wellness in the development and update of resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care.
