Job summary

Acts Retirement-Life Communities, Inc. is seeking a Wellness Home Services Licensed Practical Nurse (LPN) to provide individualized, one-on-one care to residents under an RN's direction.

The role focuses on chronic condition management, post-operative support, and safe medication administration in a compassionate setting. The ideal candidate will have an active LPN license, 1–2 years of clinical nursing experience, CPR certification, and a valid driver’s license if transporting residents.

Qualifications

• Active LPN license in good standing with state Board of Nursing.
• 1–2 years of clinical nursing experience, preferably in acute care or home health.
• Current CPR certification.
• If transporting a resident, must have a valid driver’s license and proof of auto liability insurance.

Responsibilities

• Deliver individualized, one-on-one care under supervision of an RN.
• Manage medications, monitor vital signs, collaborate with care teams.
• Educate residents and families to support safety, independence and quality of life.

Acts is currently seeking qualified candidates for our Wellness Home Services Licensed Practical Nurse (LPN) position.

In this role, you will be responsible for delivering individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN), where required by state guidelines. Specializes in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. Performs medication management, vital sign monitoring, and collaborates with multidisciplinary care teams, while providing clear education and guidance to residents and families. Requirements

The ideal candidate will meet the following requirements:

• Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
• Minimum of 1-2 years of clinical nursing experience, preferably in acute care or home health setting
• Current CPR certification (or obtain within three months of employment)
• If responsible for transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance

Pay Range

$24.38 - $36.86 / hour. Starting rate will vary based on skills and experience.
