Job summary

StoryPoint is seeking a Wellness Director to oversee clinical resident care services and lead a team in delivering high-quality care in Illinois. The role requires RN or LPN licensure and the ability to manage revenue, regulatory compliance, and resident outcomes.

The director coordinates dementia programs, care plans, and 1440 care standards while partnering with the Executive Director and Sales to market wellness. Flexibility for on-call weekends/holidays is expected.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the state preferred.
• CDP required prior to or upon hire within 90 days.
• Assisted Living/Memory Care experience preferred.

Responsibilities

• Lead, manage, and accountability for your team and their performance.
• Revenue for Wellness, Evaluations & Care Conferences and Service Plans: right care at the right price with the right staff.
• Alzheimer and Dementia Program Coordinator.
• Understand and lead to our 1440 care standards.
• Clear Communication of Vision to staff, residents, and partners.
• Community Leader Partnerships with external partners; act on items.
