Job summary

UnitedHealth Group is seeking a Wellness Coordinator to provide care coordination and administrative support for behavioral health and substance use services. You will engage members, screen for gaps in care, and help develop individualized care plans to overcome access barriers.

The role is remote in WA with one day per week in the field at Wellfound Behavioral, 3402 S19th St, Tacoma. Schedule is Monday–Friday, 8:00am–7:00pm PT, 40 hours, with telecommuting when not in the field.

Qualifications

  • High School Diploma/GED.
  • 2+ years of managed care or call center experience.
  • 1+ years in Medical/Behavioral health setting.
  • 1+ years of computer proficiency including Adobe and Microsoft Office.
  • 1+ years handling sensitive issues with PHI.
  • Live in WA with access to high-speed internet.
  • Valid WA driver's license and dependable vehicle.

Responsibilities

  • Work in an inbound/outbound member queue.
  • 25% field work to nearby inpatient Behavioral health facility.
  • Document in multiple electronic documentation systems.
  • Coordinate information to clinical team partners.
  • Provide behavioral health benefit education.
  • Contact members to screen for and identify gaps in care.
  • Provide network referrals and community resources for engagement with care.
  • Administrative support.

Wellness Coordinators function in a non-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs.

Wellness Coordinators will be a primary care navigator by locating and engaging members, screening for and identifying gaps in care, addressing SDOH needs and developing individual care plans to overcome barriers to access to care.

Wellness Coordinators will also provide administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries and verification of member demographics.

Schedule: Monday through Friday a 40‑hour work week between the hours of 8:00am to 7:00pm PST. This is a field‑based position, when not in the field will work from home.

Location: Remote in Washington. Will be required to be in the field 1 day per week (rest of week will be remote). When in the field, will be at the site below:

Primary Responsibilities

Required Qualifications
  • High School Diploma/GED.
  • 2+ years of managed care or call center experience.
  • 1+ years of experience in Medical/Behavioral health setting (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice).
  • 1+ years of computer proficiency experience to include Adobe and Microsoft Office.
  • 1+ years of experience handling sensitive issues with members and providers in a confidential manner.
  • Access to a designated quiet workspace in your home (separated from non‑workspace areas) with the ability to secure Protected Health Information (PHI).
  • Live in a location that can receive a UnitedHealth Group approved high‑speed internet.
  • Valid WA state Driver's License.
  • Dependable vehicle to drive to and from Facility visits.
Preferred Qualifications
  • Bachelor's degree in psychology, social sciences, social work, nursing, or a related field.
  • Casework experience at hospitals, social service agencies or community mental health centers.
  • Dual diagnosis experience with mental health, substance abuse, and special needs.
  • Knowledge of state waiver program.
  • Experience working in an environment that requires coordination of benefits and utilization of multiple groups and resources for patients.
  • Working knowledge of community‑based resources and services.
  • Creative problem‑solving skills with the ability to use community and network resources to meet members' needs.
  • Demonstrates initiative in achieving individual, team, and organizational goals and objectives.
  • Demonstrated creative problem‑solving skills with the ability to use community and network resources to meet members' needs.

  • Must demonstrate flexibility, ability to follow through on all tasks in a timely fashion, good attention to detail and a willingness to learn.
  • Solid oral and written communication skills—specifically telephone skills.
