• Assessing patients' conditions, developing individualized care plans, and implementing interventions as needed
• Collaborating with interdisciplinary teams to ensure seamless transitions and optimal patient outcomes
• Documenting patient information accurately and thoroughly
• Demonstrating excellent communication skills to patients, families, and healthcare teams
• Administer medications and treatments as ordered by physicians
• Performing various medical procedures, such as injections, wound care, and IV therapy
• Monitoring patients' vital signs and report any changes to the healthcare team
• Participate in quality improvement initiatives to enhance patient care and satisfactionMaintain accurate and timely records of patient interactions and care rendered
