Job summary

Silver Stream Rehabilitation and Nursing Center is seeking an experienced Registered Nurse (RN) Supervisor to oversee the nursing team during the overnight weekend shift. The role requires strong clinical leadership and the ability to work independently to ensure high-quality resident care.

Responsibilities include supervising staff, reviewing plans, ensuring regulatory compliance, coordinating with physicians, and supporting admissions and transfers.

Qualifications

• Current PA RN license.
• Supervisory or charge nurse experience preferred.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical assessment and critical-thinking skills.
• Ability to work independently and make sound decisions.
• Excellent leadership and communication skills.

Responsibilities

• Ensure residents receive high-quality care in accordance with physician orders and individualized care plans.
• Monitor resident conditions and respond to changes in clinical status.
• Provide clinical guidance and support to LPNs and CNAs.
• Address emergencies and other clinical concerns as they arise.
• Communicate with physicians and the interdisciplinary team as necessary.
• Complete and review clinical documentation, incident reports, and required follow-up.
• Assist with admissions, transfers, and other nursing responsibilities as needed.
• Ensure compliance with facility policies and state/federal regulations.

7:00 PM – 7:00 AM | 12-Hour Shifts | Weekends

Silver Stream Rehabilitation and Nursing Center is seeking an experienced and dedicated Registered Nurse (RN) Supervisor to oversee our nursing team during the overnight weekend shift. This leadership position is ideal for a strong clinician who can work independently, support nursing staff, and ensure exceptional resident care.

Responsibilities:

• Supervise and support nursing staff during the overnight shift

• Ensure residents receive high-quality care in accordance with physician orders and individualized care plans

• Monitor resident conditions and respond appropriately to changes in clinical status

• Provide clinical guidance and support to LPNs and CNAs

• Address emergencies and other clinical concerns as they arise

• Communicate with physicians and other members of the interdisciplinary team as necessary

• Complete and review clinical documentation, incident reports, and required follow-up
