Qualifications

• Current MD RN license
• Minimum 1 year of recent clinical (medical/surgical) experience
• Prior home health experience preferred but not required
• Graduation from an accredited nursing program (transcript/diploma)

Responsibilities

• Follow a designated care plan per patient needs
• Make home visits in assigned geographic territories
• Administer medications, perform wound care, treatments and procedures
• Monitor client conditions and report changes to supervisor
• Follow doctors' orders and document actions accurately
• Perform client assessments as needed and coordinate case management
• Document observations and interventions in EMR on a tablet
