Qualifications

• RN licensure in Illinois
• CCM certification or CCM preferred
• 2 years nursing experience in acute/hospital setting
• BPS CPR/BLS for healthcare provider per AHA

Responsibilities

• Conduct comprehensive assessments of medical, social, and psychological needs.
• Collaborate with providers to develop care plans.
• Coordinate referrals and home health services; manage transitions of care.
