Job summary

Reliant Care Management Company seeks a compassionate Registered Nurse to join our care team. You will assess patient needs, administer medications, monitor vitals, and collaborate with physicians and staff to ensure high-quality care.

Job Responsibilities and Duties:

• Assess patients’ health needs and develop and implement care plans

• Communicate with patients and families to provide education and support
• Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment
• Document patients’ progress and outcomes using electronic medical records
• Stay up-to-date with current research and best practices in nursing
• Participate in professional development opportunities to maintain and improve skills
• Other duties as assigned

Requirements:

• Current unencumbered state license as a Registered Nurse
