Job summary

Adaptive Home Health is seeking an elite Registered Nurse with wound care and infusion experience for full-time work in home health across multiple locations. The position follows a flexible Thursday–Monday schedule with days off Tuesday and Wednesday; 30 productivity points per week at $64.10 per point, with potential for additional pay if exceeding 30 points.

Candidates must have a current Texas RN license, at least three years of clinical nursing experience, CPR, and experience with Homecare

Responsibilities

• Conduct comprehensive home health assessments and document in the EMR.

• Supervise nursing clinical staff as needed; participate in conferences and trainings.

$85,000 - $105,000 per year approximately

• Paid $64.10 per point

Seeking elite Registered Nurses with experience in wound care and infusions! Previous home health experience highly preferred.

This position is full time with the expectation of 30 productivity points completed per week. The clinician will be paid per point and has the opportunity to earn additional pay if the clinician elects to perform more than 30 productivity points per week.

Position Requirements

Registered Nurse is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase

• Evaluate patient and home health environment and performs skilled home health assessments; clinical documentation and visit notes; care plan development; communication with physician, care team members, patient and family.
• Comfortable treating wounds and performing infusions
• Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
• Administer medications and treatments as prescribed by physician. Provides direct patient care including infusion therapy, wound care, teaching, disease management.
• Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
• Participate in regular case conference and in-services

• Competency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice Act
