Job summary

Infinite Medical P.C. is hiring a Primary Care NP/PA to provide on-site care to residents in long-term care settings in Bennington, VT. This part-time position focuses on weekends, delivering compassionate, patient-centered care to residents.

The role requires active NP/PA licensing (Vermont) and LTC experience (minimum 6 months). You will conduct exams, manage care plans, order tests, and collaborate with facility staff.

Qualifications

• Active NP/PA licensure with a Vermont license in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical and critical thinking skills.
• Passion for elder care and patient-centered care.
• Physical requirements including standing/walking and potential patient lifting.

Responsibilities

• Provide primary care services to residents in long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Maintain a safe, respectful environment for residents.
