Job summary

Cornerstone Clinical Partners is seeking a part-time Primary Care Nurse Practitioner or Physician Assistant to provide on-site care to residents in long-term care facilities in Wilkesboro, NC. This weekend position supports meaningful patient relationships and collaborative care with a dedicated team.

Applicants must hold an active NC license (NP/PA/Physician) with experience in long-term care, and demonstrate strong communication and clinical skills in a patient-centered environment.

Qualifications

• Active and unrestricted NP, PA, or physician license.
• Licensed in North Carolina with good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
