Job summary

Middleburg Rehabilitation and Nursing Center is seeking a Physical Therapist with a Bachelor’s/Master’s/Doctorate in Physical Therapy and an active state license to evaluate and treat residents. Strong communication skills and knowledge of accreditation standards are required.

You will perform physician-ordered evaluations, develop and implement treatment plans, supervise staff and aides, attend interdisciplinary and family meetings, ensure proper documentation, and provide weekend evaluations

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Active state license to practice physical therapy.
• Excellent written and oral communication.
• Knowledge of state and national accreditation standards.
• Weekend availability for evaluations.

Responsibilities

• Screen, evaluate, and treat residents needing Physical Therapy services.
• Review medical information and screenings to determine need for services.
• Perform evaluations under physician order and develop treatment plans.
• Implement plans via direct treatment, staff education, and supervision of PT staff.
• Pursue continuing education for professional growth.
• Maintain confidentiality of patient, facility, and physician information.
• Maintain daily billing information according to procedures.
• Attend interdisciplinary meetings to relay findings to rehab team.
• Attend family meetings as appropriate.
• Adhere to documentation requirements of Rehab department and facility.
• Conduct staff training via in-services.
• Follow Rehab policies and procedures manual.
• Supervise PT assistants and rehabilitation aides as needed and per state laws.
• Supervise non-professional staff as appropriate.
