Job summary

Klarus Home Health is seeking a PRN Weekend Licensed Physical Therapist to provide PT services at patients' homes in Granbury, TX and surrounding areas.

Evaluate, treat, and re-evaluate patients with home health needs; educate families and coordinate with an interdisciplinary team. Travel locally, lift up to 50 lbs, and deliver compassionate, one-on-one care in a supportive environment.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Current State license as a Physical Therapist, or multi-state PT Compact license where applicable.
• Minimum of two years of PT experience in a clinical setting; home health/geriatrics preferred.
• Excellent interpersonal and communication skills.
• Strong nursing skills as defined by standards of practice.
• Reliable transportation with valid auto liability insurance.
• Ability to work flexible hours and travel locally.
• Ability to lift up to 50 lbs. and perform physical tasks.

Responsibilities

• Evaluate patients' physical and functional abilities to develop individualized care plans.
• Plan, implement, and modify PT treatment programs based on medical referrals and assessments.
• Provide hands-on therapy, exercises, and techniques to improve mobility, relieve pain, and restore function.
• Educate patients and families on therapeutic exercises, body mechanics, and injury prevention.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated care.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and plans.
• Participate in ongoing professional development to stay current with PT trends.

Klarus Home Health is seeking a PRN / Per Diem Weekend Licensed Physical Therapist to join our team to providing PT services including assessment, treatment planning, implementation, and evaluations but primarily focusing on start of cares for our patients currently enrolled in our home health program in the Granbury, TX and surrounding areas.

Requirements

Experience:

• Graduate of an accredited School of Physical Therapy.
• Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact.
• Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred.
• Excellent interpersonal and communication skills.
• Strong nursing skills as defined by generally accepted standards of practice.
• Reliable transportation with valid and current auto liability insurance.
• Ability to work flexible hours and travel locally as needed.
• Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
