Qualifications

  • Experience coordinating patient services across multidisciplinary teams, MOUD providers, and community resource networks to deliver holistic recovery support.
  • Certified Peer. If not certified, will be certified within 18 months of hire.

Responsibilities

  • Lead Weekend Wellmobile response on weekend shifts (8:00 a.m. - 4:30 p.m.).
  • Respond to UBE WELL line calls and assess participant motivation for care.
  • Coordinate buprenorphine/MAT services and establish care transitions.
  • Respond to hospital referrals via ODSOS and conduct proactive ER outreach.
  • Travel to designated hospital facilities within 30 minutes when on-site is requested.
  • Maintain daily contact notes and submit monthly reports timely and accurately.
  • Build strong relationships with hospital staff and maintain transparent communication regarding availability.

Hours: 6-8 hour shifts between 8:00 a.m. - 4:30 p.m. Saturday and Sunday

Job Description

Nature of Work:

The Weekend Wellmobile Peer Care Coordinator is a non-clinical, peer-based position operating in a hybrid model (virtual and on-site) during assigned weekend shifts (8:00 a.m. to 4:30 p.m.) as needed. The primary focus is leading the Weekend Wellmobile response and answering calls to the UBE WELL line—assessing participant motivation, coordinating buprenorphine/Medication Assisted Treatment (MAT) services, and establishing care transitions. Because individuals located in hospital emergency rooms are already in a safe environment, ER outreach and hospital referral responses serve as complementary responsibilities. Respond to hospital referrals received via the Overdose Survivor Outreach Services (ODSOS) line and conduct proactive ER outreach. When an on-site response is requested, the position requires availability to travel and arrive at designated hospital facilities within 30 minutes. Building strong relationships with hospital staff and maintaining transparent communication regarding availability are critical.

Minimum Qualifications

Education:

Associate degree in behavioral health or related field from an accredited college or university.

Experience:

Experience coordinating patient services across multidisciplinary teams, MOUD (Medications for Opioid Use Disorder) providers, and community resource networks to deliver holistic recovery support. Experience working with individuals on medications for opioid use disorders. Must be able to maintain daily contact notes and submit monthly reports timely and accurately. Must be able to coach and model effective coping skills and develop a relationship of rapport and trust. Strong communication skills with other health professionals, coworkers and clients. Basic computer skills to include Microsoft Word and Excel. Professional demeanor.

License or Certification:

Certified Peer. If not certified, the expectation is that the applicant will be certified within 18 months of hire.

Desired or Preferred Qualifications:

Ability to establish and maintain effective working relationships with other employees and the public. Strong knowledge of the Anne Arundel County substance use disorder system of care and resources.
