Job summary

Optimal Care is seeking a Triage Registered Nurse to serve as a critical clinical connection point across Home Health, Hospice, and Physician Services. You will respond to patient needs, guide care decisions, coordinate services, and facilitate clear communication among patients, families, providers, and care teams throughout the healthcare journey.

This role is ideal for an experienced RN who excels at balancing urgency with compassionate care, and who can lead with confidence to impact patient

Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State License as a Registered Nurse in Michigan, Indiana, and Ohio.
  • Minimum 2 years experience as a registered nurse in an appropriate clinical care setting.
  • Minimum 1 year of supervisory or leadership experience.

Responsibilities

  • Assess patient needs and coordinate care plans across Home Health, Hospice, and Physician Services.
  • Provide after-hours clinical leadership and support field staff.
  • Facilitate communication between patients, families and care teams.
  • Contribute to staff education and onboarding with clinical expertise.

About this Role

Serve as a critical clinical connection point across Home Health, Hospice, and Physician Services, ensuring patients receive timely, coordinated, and high-quality care. As a Triage Registered Nurse, you'll respond to patient needs as they arise, guiding care decisions, coordinating services, and facilitating clear communication between patients, families, providers, and care teams throughout the healthcare journey. As a key clinical resource, you'll help ensure every patient receives the right level of care at the right time while supporting continuity, safety, and positive outcomes.


This role is ideal for an experienced RN who is confident in clinical decision-making, thrives in a fast-paced environment, and excels at balancing urgency with compassionate care. You'll assess patient needs, coordinate care plans, provide after-hours clinical leadership, and support field staff to ensure safe, effective care delivery across all service lines. You'll also contribute to staff education and onboarding by sharing clinical expertise and reinforcing best practices. Your ability to think critically, communicate effectively, and lead with confidence will directly impact patient safety, operational excellence, and the experience of the patients and families we serve.

    • Jackson, MI

    • Grand Rapids, MI

    • Bingham Farms, MI

    • Saginaw, MI

    • Kalamazoo, MI

    • Maumee, OH

    • Carmel, IN



  • Main Service Area: Must live near one of these office locations. These roles work mostly remote but do go out on patient calls from time to time.


Hours


  • Office Hours: Saturday to Sunday, 8 a.m. to 8 p.m.


Required Qualifications


  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing

  • Current State License as a Registered Nurse in Michigan, Indiana, and Ohio

  • Minimum 2 years experience as a registered nurse in an appropriate clinical care setting

  • Minimum 1 year of supervisory or leadership experience

  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Pay Range

$31 - $34 USD
