Harbors Home Health & Hospice is seeking a weekend & on-call RN to join our Grays Harbor and Pacific County team in Washington. This role combines daytime home health visits with overnight hospice on-call coverage to ensure continuity of care for patients and families over the weekend.

The position emphasizes compassionate, clinically confident nursing support and collaboration with caregivers, providers, and the broader care team to respond to urgent needs promptly.

Qualifications

• WA state RN license required.
• CPR/BLS certification required.
• Valid driver’s license with acceptable record.
• Ability to work independently and respond to urgent clinical situations.
• Strong assessment, communication, organization and critical-thinking skills.

Responsibilities

• Provide daytime home health visits and hospice on‑call coverage Friday–Sunday.
• Respond to calls, assess patient needs, provide triage and in-person visits when needed.
• Collaborate with the team, providers, pharmacies, facilities, caregivers and families.
• Participate in admissions or other visits as needed.

Are you an experienced RN looking for a 3 day schedule while making a meaningful difference for patients and their families?

Harbors Home Health & Hospice is seeking a compassionate and clinically confident weekend & on-call RN to join our team.

This unique position combines daytime patient visits with overnight hospice on-call coverage, providing continuity of care and ensuring our patients and families have access to experienced nursing support throughout the weekend.

SCHEDULE:

Friday-Sunday

Daytime: Provide scheduled home health/hospice patient visits throughout the service area.

Evenings & Nights: Provide hospice on-call coverage for urgent patient and family needs.

Respond to calls, assess patient needs, provide telephone triage and clinical guidance, and make in-person visits when necessary.

Collaborate with the team, providers, pharmacies, facilities, caregivers, and families.

Participate in admissions or other visits as needed.

REQUIRED:

• Current WA State RN license


• Current CPR/BLS certification


• Valid drivers license with acceptable drivers abstract
