Job summary

LIFE Pittsburgh is seeking an on-call Registered Nurse to support Participants in our Adult Day Health Centers. You will perform telephonic and in-person assessments as needed, triage, and administer treatments and medications to ensure quality care.

You will coordinate Participant assessments and plans of care, communicate with physicians, and document per regulations, with travel between LIFE Pittsburgh centers as required.

Qualifications

• Graduate of an accredited school of nursing (Bachelors preferred).
• Minimum 1 year experience working with the frail or elderly.
• Experience working with and relating to the geriatric population and their family members.

Responsibilities

• On-call phone calls and communication involving Participant care through our on-call system.
• Use clinical knowledge to determine best course of action when receiving on-call calls.
• Ensure high quality care and communication is delivered at all times.
• Provide direct care to patients, including triage, administering treatments and medications.
• Assist in development, follow and coordinate each Participant comprehensive assessment and plan of care.
• Notify physician when necessary of changes in the patient's condition, weight, medication errors, and other observations.
• Complete all necessary documentation according to regulations and LIFE Pittsburgh policies.
