Bridges-Hospice in Monroeville, PA seeks an experienced RN to deliver compassionate hospice care across patients and families. Qualifications

Job Summary:In this position you will respond to calls from patients, families, caregivers, and referral sources. Thenurse assesses, utilizes critical thinking and gives advice or delivers care to patients utilizing the nursingprocess of assessment, planning, interventions, implementation, and evaluation; and effectively interactswith patients, significant otheIS, and other interdisciplinary team members while maintaining standar~ ofprofessional nursing and clinical competency. This position reports to the Patient Care Coordinator.Job Duties:Effectively performs nursing assessment, research changes in patient health or pain in coordination withphysicians.Responding to calls from answering service and supervisors in a timely manner.Triaging patient-related calls by determining the needs of the caller and then responding in an appropriatemanner either by phone or with a visit.Implementing changes to the plan of care as needed with interdisciplinary collaboration with the supportof the hospice medical director and /or the attending physician.Documenting and communicating all calls, admissions, interventions, care plan changes, physiciancontacts in an effective manner.Documenting accurate assessment of patient status and updating the plan of care utilizing appropriatecommunication (verbal, electronic, written, email documents and databases).Document patient care reflecting nursing interventions, patient response to care, focusing on the patient'sgoals.Documentation includes evidence of appropriate patient/significant other teaching, and the understandingof these instructions is noted in the medical record.Assisting patients, family members or caregivers wjth concern and empathy; respect confidentiality andprivacy and communicate in a courteous and respectful manner.Attending patient deaths to provide support to the patient's family, contact the funeral home, disposal ofnarcotics, notify the IDT as well as contracted vendors providing services to the patient.Provide bereavement resources to the family as appropriate.Responds to referrals calls, providing the caller with infonnation regarding hospice care, screening callsfor appropriateness, taking patient information and coordinating with the on -call supervisor to schedulethe admission visit.Performs patient admissions assigned by supervisor.Reporting concerns the hospice supervisor on-call.Follow the On-call expectations for triage and visit calls.Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques andsymptom control.Provides direct patient care, pain symptom managementto patients as ordered in the patient's plan of careto maintain the highest level of comfort and quality of lifeEducates patient and patient's family reganling death and dying, individual responses to death and patientrights.Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.Offers ongoing support and palliative care.Follow six medication rights and reduce the potential for medication errors.Initiates needed referrals to other disciplines and/or community resources.May supervise hospice aides as required by the Medicare COPs.Follows up with obtaining lab results and notifying physician of any significant results.Notifies supervisor of patients who need to be seen on the following business day and submits updatednursing plan to plan RN Case Manager.Assist with training and educating new hires.Communicates changes in patient condition to PCP and IDT.Maintains current knowledge of nursing practices and asks for guidance when needed.Understands Hospice philosophy and issues of death and dying.Adheres to all organization policies and procedures.Attends Mandatory Staff meetingsCompletes Documentation within 48 hours (about 2 days).Understands record keeping requirements regarding billing and documentation per regulations concerningpatient care.Signs in and out of facilities and documents in appropriate chart(s).Maintains a good relationship with facilities and reports off to appropriate staff.Participates in the Bridge Program.Represents the organization in a positive and professional manner in the community.Other duties, responsibilities, or activities may change with or without notice.Professional Requirements:Adheres to dress code, appearance is neat and clean.Completes annual educational requirements as assigned.Maintains patient confidentiality at all times.Reports to work on time and as scheduled, completes work within designated time.Wears identification when in community.Communicates the mission, ethics and goals of the organization.Education/Regulatory Requirements:High School Diploma or G.E.D.Graduate of an accredited school of Nursing.Current Registered Nurse license in the State of Pennsylvania and is listed in good standing at time of hireand at time of renewal.Basic computer knowledge.Comprehensive knowledge of general nursing theory and practice.At least 2 years of similar experience preferred.Current TB screening.Travel Requirements:Travel is required throughout the work day. Valid state driver's license and reliable automobile,current automobile insurance and be willing to operate personal car necessitated by nature of job.Provides copy upon renewal of driver's license and automobile insurance.Language Skills:Able to communicate effectively in English, both verbally and in writing.Physical Demands:The position requires sitting, standing, walking, bending, kneeling, and stooping, crouching, reaching,and climbing, use of telephone as well as computer throughout most of the day. The employee must haveadequate (with correction) close vision, distance vision, depth perception, and the ability to focus.The employee must frequently lift or move objects and patients weighing over 50 pounds.
