Job summary

OnSite Medical Solutions seeks a Nurse Practitioner to perform consultations in nursing homes, assisted living facilities, SNFs, and home settings. You will receive full administrative support and clinical mentorship, enabling autonomous, private-practice-style scheduling with a collaborative team.

You will deliver high-level medical and mental health care to older adults, with emphasis on patient-centric care, education, and evidence-based interventions.

Qualifications

Responsibilities

• Round at assigned facilities and manage acute, subacute, and primary care needs of older adults.

• Proactively follow up with patients and assess intervention efficacy.
• Provide education on best practices and non-pharmacological options when possible.
• Integrate interventions to optimize patient outcomes.
