Job summary
Vitae Health is seeking a dedicated Weekend Nurse Practitioner to provide high-quality, patient-centered care for residents in a skilled nursing facility every weekend (Sat-Sun).
The NP will assess, diagnose, and manage acute and chronic conditions, collaborating with an interdisciplinary team to improve patient outcomes and quality of life. Requires NP board certification, active APN license, and DEA registration.
Qualifications
- Master's degree in Nursing from an accredited program.
- Active APN license in the respective state.
- Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
- Current DEA registration and prescriptive authority (or eligibility).
Responsibilities
- Perform comprehensive health assessments and manage conditions with individualized care plans.
- Order/interpret diagnostics, prescribe treatments, review medications and monitor status.
- Collaborate with physicians and interdisiplinary team to improve outcomes.
- Support value-based care, reduce avoidable hospitalizations, and guide performance improvement.
- Facilitate smooth transitions of care and ensure continuity across settings.
