Job summary

Clovernook Healthcare, LLC is seeking a Weekend Warrior LPN for a night shift in Cincinnati, OH. You will deliver safe, therapeutic resident care from admission through discharge in collaboration with physicians and a multidisciplinary team.

Responsibilities include medication administration, assessments, wound care, and documentation, while supervising Nursing Assistants and communicating changes to the physician as needed. Strong LTC experience is preferred.

Qualifications

• LPN credential with an active Ohio license and CPR certification.
• Minimum of 1 year experience in Long Term Care.
• Graduate from an accredited nursing program.

Responsibilities

• Provide direct nursing care per the resident care plan, including meds, assessments, wound care, and documentation.
• Supervise day-to-day nursing activities performed by Nursing Assistants.
• Monitor patient condition and notify the physician of changes.
