CareCore Health LLC is seeking a dedicated Night Shift Supervisor in Mentor, OH. Experience as an LPN or RN with leadership responsibilities is required, with a weekend night schedule designed for consistency.

Responsibilities include supervising weekend night shifts, mentoring staff, and ensuring quality resident care while complying with regulations. Emphasis on communication with physicians, residents, families, and the interdisciplinary team.

Qualifications

• Current Ohio LPN or RN license in good standing.
• Previous long-term care or skilled nursing experience required.
• Supervisory or charge nurse experience preferred.
• Strong clinical assessment, leadership, and communication skills.
• Experience with PCC is a plus.

Responsibilities

• Provide clinical leadership and supervision during weekend night shifts.
• Support and mentor nursing staff while promoting a positive team environment.
• Ensure high-quality resident care and compliance with state and federal regulations.
• Respond to changes in resident conditions and assist with clinical decision-making.
• Communicate effectively with physicians, residents, families, and interdisciplinary team members.
• Complete documentation accurately and ensure PCC documentation is maintained.
• Participate in admissions, incident follow-up, and other clinical responsibilities as needed.
• Serve as the nursing leader on duty during assigned shifts.
