Job summary

Constellation Health Services is seeking two Weekend Licensed Practical Nurses to join our team in the Orange, CT area. You will provide in-home nursing care, follow the care plan, and work with a multidisciplinary Hospice team to support patient comfort and family education.

The ideal candidate is a graduate of an accredited LPN program, licensed in CT, with 1–2 years’ LPN experience, and a willingness to work alternating weekends and holidays as needed.

Qualifications

• Graduate of an accredited practical nurse program.
• Licensed LPN in the state of employment
• At least 1–2 years of LPN experience; home care preferred
• Experience with end-of-life care considerations is a plus

Responsibilities

• Provide nursing care per state Nurse Practice Act in home/community settings.
• Follow the plan of care and document services accurately.
• Educate patient and family about diagnosis and care plan.
• Coordinate with Hospice IDT and other team members to meet patient needs.
