Responsibilities

• Performs a comprehensive assessment on all assigned patients with complete documentation.
• Communicates changes in patient condition promptly to the care team.
• Uses nursing diagnoses and assessment data to set measurable plan-of-care goals and updates as needed.
• Identifies and prioritizes patient needs and documents them in the care plan.
• Collaborates with patient, family, and health-care team for planning and discharge planning referrals.
• Evaluates patient response to interventions and adjusts the plan of care accordingly.

Education

Bachelor of Science Nursing

Registered Nursing license issued by the State of Illinois
