Apploi is seeking a Nurse Practitioner (Skilled Nursing) to lead internal medicine services at a Skilled Nursing Facility on Saturdays and Sundays. You will collaborate with an interdisciplinary team to minimize weekend hospitalizations and manage the care of short- and long-term residents under physician guidance.

You will perform comprehensive exams, develop patient management policies, and provide advanced care planning with a focus on infectious disease.

Qualifications

• APN licensure in Florida and active DEA number.
• Master's degree required.
• Strong clinical background in Infectious disease.
• Excellent written and verbal communication skills.
• Ability to thrive in a fast-paced environment.

Responsabilités

• Act as the SNF-ist, lead internal medicine services on weekends.
• Collaborate with interdisciplinary team and physicians to reduce hospitalizations on weekends.
• Work with residents and nursing staff under medical directors and attending physicians.
• Evaluate new admission visits.
• Coordinate discharge visits.
• Evaluate patients with changes in condition.
• Provide care including Advanced Care Planning and comprehensive health assessments.
• Prescribe medications under protocol and manage acute/chronic illness.
• Perform comprehensive physical examinations and develop management plans.
• Facilitate referrals to other health professionals and facilities.
• Communicate with attending physicians and specialists regarding patient care.

Nurse Practitioner (Skilled Nursing)

Major Responsibilties

• As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services on Saturday and Sunday.

• You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitilizations on weekends.

• You will work with the entire facility's patient population - short and long-term residents with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians

• Evaluation of new-admission visits

• Coordination of discharge visits

• Evaluations for patients exhibiting changes in condition

• As a SNF-ist, you will provide types of care, including but not limited to:

• Advanced Care Planning

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

• Prescribe all necessary medications and treatments

• Perform comprehensive physical examinations of patients

• Develop and implement patient management policies and procedures

• Facilitate referrals to other healthcare professionals and medical facilities

• Communicate with attending physicians and/or specialists regarding patient care

Qualifications

• Advanced Practice Licensure (APN) with a Florida license

• Must have an active license and DEA number.

• Strong clinical background in Infectious disease.
