We are seeking a part-time LPN to work 8-hour shifts on Friday, Saturday, and Sunday from 12:00 a.m. to 8:00 a.m.. This role provides backup on-call support to our nursing team and involves travel across the service area to healthcare facilities, assisted living homes, patients’ homes, and the home office when necessary. The ideal candidate is calm under pressure, confident in decision-making, and comfortable working independently while collaborating with a broader interdisciplinary team.

Key Responsibilities

• Triaging after-hours and weekend hospice calls to ensure timely and appropriate interventions
• Supporting the intake and referral process with compassion and efficiency
• Completing in-person field visits when necessary to assess patient needs
• Accurately documenting in the EMR system and communicating with the care team
• Providing holistic, patient-centered care with sensitivity to emotional, physical, and spiritual needs

Qualifications:

• Current LPN license in PA
• Hospice experience required
• Experience with EMR systems required
• Proficient with computers and digital tools
• Strong interpersonal and communication skills
• Ability to work independently and prioritize tasks
• Reliable transportation, valid driver’s license, and auto insurance for travel within the service area
