Job summary

Anchor Health is seeking an Admissions Registered Nurse to present our hospice services to patients and families and to admit appropriate patients. The nurse will coordinate with physicians, admission staff, and the care team to ensure smooth transition to home-based hospice care.

With responsibilities ranging from information gathering to admission orders, the role requires strong communication, clinical knowledge, and travel between Sacramento and Solano County.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in State of CA and CPR certification.
• RN license or baccalaureate degree in nursing from an NLN-accredited program with active license.
• Minimum of one year of nursing experience within last three years; OR a BSN with current RN license.
• Two years of experience in public health, home health, or hospice nursing preferred.

Responsibilities

• Present Anchor Health services and philosophy to patients and families to admit appropriate patients.
• Evaluate patients for hospice appropriateness based on medical records.
• Obtain agreement for admission of referred patients meeting criteria.
• Analyze data to facilitate admission orders.
• Build relationships with referral sources and internal teams to ensure high-quality admissions.
• Maintain clinical and professional competence.
• Educate patients and families about hospice and palliative care.
