Job summary

Frontier Home Health is seeking a Registered Nurse to provide high-quality, patient-centered care in the home setting in Colorado Springs, CO, primarily on weekends. You will manage patient care plans, perform skilled nursing visits, and support independence and safety at home.

Ideal candidates hold an active RN license, 1+ year of nursing experience (home health preferred), CPR, a valid driver’s license, and excellent communication and time management skills.

Qualifications

• Active and unrestricted RN license required.
• 1+ year nursing experience; home health preferred.
• Availability for patient home visits on Saturdays and Sundays.
• CPR certification required.
• Valid driver’s license, reliable transportation.
• Strong communication and time management skills.

Responsibilities

• Provide skilled nursing visits per plan of care.
• Perform comprehensive patient assessments and monitor status.
• Coordinate with physicians, therapists and care team.
• Document accurately in the EMR system.
• Educate patients and families on disease management and safety.
