Qualifications

Responsibilities

• Perform Start of Care comprehensive assessments and timely OASIS.
• Draft care plans, reconcile patient medications, and verify orders for care.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide reports to the Case Manager.
• Provide health care instructions to patients and families as per assessment.
