Job summary

Neogen Care is seeking a Weekend Home Health Registered Nurse to provide direct patient care in patients' homes in Modesto and surrounding areas. Qualifications

• Minimum Associate degree in Nursing (RN).
• Current California RN license and CPR/BLS certification.
• 2–3 years of RN home health experience preferred.
• Wound care, wound vac, IV/TPN, and medication management experience.
• Experience charting with EMR software (Wellsky or Kinnser a plus).

Responsibilities

• Provide direct patient care in the home under physician supervision.
• Follow start of care plans, discharge planning, and written care plans.
• Submit regular progress reports to attending physician and clinicians.
• Discharge patients after physician/clinical manager approval.
• Coordinate plan of care with nursing team and resources.
