Job summary

Qualifications
- LPN license in Pennsylvania.
- Minimum of one year recent LPN experience.
- Graduate from an accredited nursing program.

Responsibilities
- Make home visits to clients in designated Berks County areas.
- Administer medications, wound care, treatments, and procedures.
- Monitor client conditions and report changes to supervisors.
- Follow up on doctors' orders and document actions.
- Perform client assessments and coordinate case management.
- Document observations and care in electronic medical records on a tablet.
