Job summary

Qualifications

• Must hold current DFS certification.
• High school diploma or GED preferred.
• Excellent communication skills and physical ability.
• Empathetic, caring demeanor toward patients.

Responsibilities

• Provide personal care and activities of daily living.
• Assist with transfers, repositioning, and ambulation.
• Support basic nursing tasks under supervision.
• Prepare meals in line with dietary needs.
• Offer companionship and engage clients in activities.
• Perform light housekeeping and document care using EVV.
• Maintain confidentiality and report changes per care plan.
• Travel within service area to visit clients.
