Qualifications

Responsibilities

• Provide in-home care and assistance to clients, including personal care, medication management, and daily living activities.
• Utilize Hoyer lifts and other assistive devices to ensure safe and comfortable transfers.
• Monitor and report changes in client condition, vital signs, and medication adherence.
• Develop and implement personalized care plans in collaboration with clients, families, and healthcare providers.
• Maintain accurate and detailed records of client care, including documentation and billing.
• Collaborate with other healthcare professionals, including nurses, therapists, and social workers, to deliver comprehensive care.
