Qualifications

• High school diploma or equivalent.
• Valid driver’s license and reliable transportation.
• Pass background checks and drug screenings as required.

Responsibilities

• Assist individuals with personal care and daily living activities.
• Support participation in community outings, recreational activities, and social events.
• Provide transportation to medical appointments, vocational programs, and community activities.
• Assist with meal preparation, household chores, and maintaining a safe living environment.
• Implement individualized service plans and supporting personal goals.
• Provide supervision and ensuring the health, safety, and well-being of individuals served.
• Complete required documentation, daily notes, and electronic records accurately and timely.
• Observe and reporting changes in physical, emotional, or behavioral health.
• Respond to emergencies and apply CPR/First Aid procedures when necessary.
• Maintain confidentiality and adhere to HIPAA, DDS, and agency policies.
