Job summary

CONSUMER DIRECTED SERVICES INC. seeks a Certified Nursing Assistant (CNA) to provide weekend home care services with shifts from 08:00 AM to 2:00 PM. You will assist with daily living activities, mobility, meals, and companionship while following the care plan and ensuring safety.

Responsibilities include maintaining accurate records, reporting changes, and protecting patient confidentiality in a supportive home environment.

Qualifications

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills and ability to follow instructions.
• Physical ability to lift approximately 50 lbs, bend, stand, and perform necessary tasks.
• Compassionate attitude toward patient care.
• This role requires dependability, adaptability to client and family needs, and a team-oriented approach to deliver quality home care.

Responsibilities

• Providing direct patient care and assisting with daily living activities such as personal hygiene, bathing, grooming, and mouth care.
• Assisting with transfers, repositioning, and ambulation to ensure patient mobility and safety.
• Administering basic nursing care and prescribed exercises under supervision.
• Preparing meals according to client preferences and dietary needs.
• Offering companionship and engaging in activities tailored to individual interests.
• Performing light housekeeping duties and maintaining accurate records of care provided.
• Monitoring and reporting changes in patient condition, adhering to the care plan, and documenting tasks via EVV app or time sheet.
• Upholding patient confidentiality, respecting patient rights, and ensuring safety by reporting concerns such as abuse or neglect.
