Trilogy Home Healthcare, a Humana company, is seeking a Clinical Team Manager, RN for Home Health in Tampa, FL. This full-time role in a dynamic, team-driven environment offers in-office days Friday to Monday with WFH eligibility on weekends.

The incumbent will oversee a clinical interdisciplinary team, ensure quality care, and collaborate with physicians to resolve patient care issues. Ideal candidates have RN licensure, 1+ year Medicare Home Health experience, and strong leadership.

• Graduate of an accredited nursing or therapy program; must be a licensed RN, PT, OT, or SLP
• 1+ year of Medicare Home Health field experience; Experienced field clinicians encouraged to apply
