Qualifications

• Pharmacist license in the US state of Texas is required.
• Experience in hospital pharmacy is preferred but not mandatory.
• Pharmacist license in another state requires ATT authorization from the Texas Board.

Responsibilities

• Consult on medication orders with nursing and medical staff.
• Validate patient histories and medication profiles.
• Monitor drug therapy and perform drug regimen reviews and reconciliations.
• Ensure compliant documentation and reporting per policy.
