This role includes conducting patient assessments, developing emergency response plans, coordinating transfers, and guiding nursing staff to deliver compassionate, compliant, and organized care.

Qualifications

• Current RN license in the state of Colorado.
• Minimum of 0 years of experience in a long‑term care or rehabilitation setting.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team; excellent patient care.
• Strong organizational and time management skills.

Responsibilities

• Oversee delivery of patient care on weekends.
• Develop and implement individualized care plans, in collaboration with other members of the interdisciplinary team.
• Conduct patient assessments, identify areas of concern, and develop emergency response plans as needed.
• Provide guidance and support to our nursing staff, ensuring that they have the resources and training they need to provide excellent care.
• Coordinate patient transfers and discharges, ensuring a smooth transition for our patients and their families.
• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
• Collaborate with other departments, such as therapy and dietary, to ensure that patient needs are met and that care is coordinated.
