Job summary

Friendly Faces Senior Care is seeking qualified social work professionals to provide counseling and coordinate services for clients with mental health concerns, family dynamics, and crisis interventions. The role supports advocacy within healthcare settings and collaboration with multidisciplinary teams.

Required MSW from an accredited program; LMSW/LCSW preferred but not mandatory for all roles. Evening/weekend shifts may be needed based on client availability.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited program required.
• LMSW or LCSW preferred but not mandatory for all positions depending on role requirements.
• New graduates are encouraged to apply for certain positions.
• Strong interpersonal skills conducive to building rapport with clients.
• Knowledgeable about social justice issues affecting individuals/families within the community.
• Proficient use of electronic health record systems.
• Flexible problem-solving abilities guided by empathy.

Responsibilities

• Provide counseling support to clients dealing with mental health concerns, family dynamics, crisis intervention, or substance abuse.
• Advocate for clients’ rights within healthcare settings or community resources.
• Coordinate services with other professionals including healthcare providers, governmental agencies, schools, and community organizations.
• Maintain accurate records of client interactions including assessments, case notes, treatment plans in compliance with ethical guidelines and legal regulations.
• Stay informed about local resources available for clients (e.g., housing assistance programs; mental health services) to provide appropriate referrals.
• Participate in multidisciplinary team meetings to discuss patient care strategies.
• This role may require working evenings or weekends based on client availability.
