Job summary

BAYADA Home Health Care is seeking a compassionate Weekend Baylor Registered Nurse (RN) to join our in-home care team in Chester County, PA. This full-time position serves adult and geriatric clients on a per-visit basis, with a weekend schedule of Friday–Sunday or Saturday–Monday.

A PA RN license and at least one year of recent clinical experience are required; home health experience is preferred. You may visit clients in designated territories and document care using EMR tools.

Qualifications

• Requires PA RN license and graduation from an accredited nursing program.
• Minimum of one year of recent clinical (medical/surgical) experience.
• Prior home health experience preferred but not required.

Responsibilities

• Follow designated care plans based on patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Document doctors' orders and client observations accurately.
• Perform client assessments and coordinate case management.

This office services our adult and geriatric clients on a per visit basis throughout Chester County.

Weekend schedule would be Friday - Sunday OR Saturday - Monday.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$60.00 - $73.00 per point
