Qualifications

Responsibilities

• Perform patient evaluations (initial, progress, discharge) and create treatment plans.
• Provide evidence-based treatment and adjust goals with patient input.
• Lead education/mentorship activities for staff and students.
• Develop and evaluate standards of care and outcomes analyses.
• Maintain accurate patient documentation and compliance with audits.
• Support quality, service, and productivity targets.
• Lead program development and participate in training for staff.
• Coordinate outreach activities and in-services as required.
• Complete additional duties as assigned.
